Home Care RN Case Manager | PRN

About the role

The Home Care RN at Peterson Health is responsible for performing admission of patients, recertification’s, resumption of care, supervisory and routine visits. They implement the plan of care for patients, attend Case Conferences and Weekend Reports, and document OASIS data sets. They provide care as per departmental policies and procedures and any additional duties assigned by the supervisor.

Requirements

• Valid RN License in State of Texas.
• Current American Heart Association CPR Certificate.
• Valid Driver’s License and Car Insurance.
• Minimal 1 year in medical setting experience preferred with 1-5 years home health experience.

Qualifications

• Bachelor's Degree in Nursing preferred.

Skills

• Strong communication skills.
• Ability to work independently and as part of a team.
• Proficiency in using electronic health records (EHR).
