HOME CARE REGISTERED NURSE

About the role

Liberty Home Care provides recovery support at home, enhancing patient outcomes after illness, trauma, or surgery. They seek an experienced Home Care Registered Nurse (RN).

Responsibilities

• Participates in interdisciplinary care planning.
• Provides professional nursing services according to the plan of care.
• Promotes favorable outcomes through collaborative practice and resource management.
• Accurately documents services provided, facilitates coordination, justifies reimbursement, and supports quality improvement.
• Demonstrates positive interpersonal relationships.
• Achieves work assignments and engages in professional development.

Requirements

• Successful completion of nursing education from an approved school of nursing.
• Current RN licensure in the state where the branch is located.
• At least one year of clinical nursing experience, preferably in an acute care or long-term setting, with home health experience.
• Knowledge of fundamental medical-surgical nursing principles and practices.
• Excellent clinical nursing skills.
• Understanding of adult learning principles.
• Knowledge of medical recordkeeping standards.
• Effective communication skills, both oral and written.
• Computer proficiency and knowledge of various applications.
• High-level problem-solving, critical thinking, and reasoning abilities.
• CPR certified.
