Home Care Program Care Manager

What You'll Do

• Provide oversight and direction to assigned staff
• Oversee all care management aspects of HC program
• Contribute to the development and implementation of procedures, standards and guidelines for program/department operations
• Ensure program integrity and regulatory compliance
• Implement quality measures' conduct quality reviews to ensure they are met
• Participate in planning and execution of comprehensive staff development and training programs
• Participate in program analysis and evaluation including determination of service needs/gaps and the quality and effectiveness of services delivered
• Advises and consults with other HC management staff from various disciplines on a regular basis
• As directed, represents OCES at external meetings and participates in public outreach/education activities and events
• Maintain a detailed knowledge of current OCES policies, and standards, funding source regulations
• Maintain accurate documentation

What You'll Bring

• Your passion for helping others and desire to make a difference
• Adaptability to be able to change as the needs of consumers or programs change
• Minimum of five years of experience in the human services field; including two years in a supervisory capacity
• ASAP experience preferred, experience in organization, administration and supervision
• Excellent written and verbal skills
• Excellent computer skills
• Proficient with Microsoft Office Suite or related software
