Primary Functions
- Aids in the admission of the client to the Agency by conducting initial and ongoing assessments, including OASIS assessments at appropriate times.
- Interprets Physical Therapy Agency services to clients and families, assisting with care planning.
- Makes arrangements for out-patient services as needed.
- Evaluates the effectiveness of Physical Therapy intervention and updates the client care plan as necessary.
- Instructs clients and their families in the use of prosthetic, orthotic, and assistive devices.
- Records and reports the client's reaction to treatment and any changes in condition to the physician.
- Communicates with the physician about changes in the client's status and obtains additional orders, if necessary.
- Communicates with other team members, including Home Health Aides, and instructs them in certain phases of physical therapy.
- Develops an assessment of the client's condition and prepares periodic evaluations and summaries.
- Plans and writes clinical/progress notes for each visit, incorporating them into the client's record within 72 hours.
- Supervises the home health aide every two weeks when physical therapy is the sole professional discipline involved in the client's care.
- Supervises the Physical Therapy Assistant at least once monthly, as outlined in the physical therapy assistant job description.
Qualifications
- Associate's degree (A. A.) or equivalent from a two-year College or technical school.
- At least 2 years’ Physical Therapy experience.
- Registered Physical Therapist in the State of Florida.
- CPR certification.
