HOME CARE OCCUPATIONAL THERAPIST OT

About the role

Liberty Home Care provides recovery support at home, enhancing patient outcomes after illness, trauma, or surgery. We seek an experienced HOME CARE OCCUPATIONAL THERAPIST (OT) to join our team in Greenville, NC.

Responsibilities

• Provides patient care services and direct treatment
• Evaluates patients with physician orders, using various evaluations
• Develops a care plan based on patient assessment
• Documents patient care data accurately and in a timely manner
• Coordinates with Certified Occupational Therapy Assistants (COTAs)
• Communicates with internal and external contacts
• Discusses reimbursement policies and treatment plans
• Coordinates referrals to other disciplines
• Conducts in-service education and supports staff development
• Assists with employee orientation

Requirements

• Minimum bachelor’s degree in occupational therapy from an accredited school
• Successfully completed the National Certification Exam
• Current licensure from the state where therapy will be provided
• One year of therapeutic experience in a non-home health care setting preferred
• Strong knowledge of occupational therapy principles and patient evaluations
• CPR certified
• Excellent communication and organizational skills
• At least 12 hours of continuing education annually
• Willingness to travel and work under varying conditions
