HOME CARE OCCUPATIONAL THERAPIST OT

About the role

Liberty Home Care offers recovery services at home to improve patient outcomes after illness, trauma, or surgery. We seek an experienced HOME CARE OCCUPATIONAL THERAPIST (OT).

Responsibilities

• Provides patient care services and direct treatment.
• Evaluates patients based on physician orders, using various evaluations.
• Develops a personalized care plan based on patient assessments.
• Documents patient care data accurately and in a timely manner.
• Collaborates with Certified Occupational Therapy Assistants (COTAs) and provides supervision.
• Schedules patient visits and communicates with relevant parties.
• Communicates with patients, families, and other healthcare professionals about treatment plans and company policies.
• Coordinates referrals to other healthcare professionals as needed.
• Conducts in-service education and supports staff development.
• Participates in ongoing education and research activities.

Requirements

• Minimum bachelor’s degree in occupational therapy from an accredited school.
• Successfully completed the National Certification Exam.
• Current licensure in the state where therapy will be provided.
• One year of therapeutic experience in a non-home health care setting preferred.
• Strong knowledge of occupational therapy theories and concepts.
• CPR certified.
• Excellent communication and organizational skills.
• A minimum of twelve hours of continuing education annually.
• Willingness to travel and adapt to various work conditions.
