Home Care Nurse - RN

The client is a 19‑year‑old male with traumatic brain injury (TBI), wheelchair‑bound, limited speech, hemiplegic; neurological and/or behavioral health experience is a must.

Responsibilities

• Medication set up and oversight
• Provide thoughtful care to clients based on a personalized Plan of Care
• Provide in‑home assessments
• Monitor and track changes in the client’s conditions and needs, and keep the Director of Nursing and other appropriate personnel informed
• Collaborate with the client’s family on plan of care, health changes, and concerns

Requirements

• At least 1 year nursing experience with neurological and/or behavioral health
• At least 1 year nursing experience in home care
• Current unrestricted RN license in Utah
• Certified CPR certification
• Negative TB skin test or chest X‑ray
• Ability to pass a drug test and background check
• Licensed driver with valid automobile insurance
• Physical ability to stand, walk, squat, bend, kneel, reach, twist, push, pull, and lift up to 75 pounds

Schedule

Available shifts in Park City:

• Monday – Thursday: 2:30 pm – 7:00 pm
• Friday, Saturday and Sunday: 1:00 pm – 7:00 pm
• Saturday, Sunday & Monday: 7:00 pm – 7:00 am
