Home Care Nurse Manager

About the role

Piedmont Health Senior Care offers the Program of All-inclusive Care for the Elderly (PACE) to enhance the lives of seniors in the community. We provide comprehensive, personalized healthcare and related services to help seniors maintain their independence at home.

Responsibilities

• Ensures all home care in-home aide requirements for nursing are met for the Piedmont Health Home Care agency.
• Develops and provides education to in-home aides.
• Collaborates with the Home Care Manager to ensure compliance, high quality, and efficient operations.
• Works collaboratively with the PACE team on coverage of home care services, quality, and effective communication between the two entities and with PACE participants and families.
• Maintains policies and procedures for home care.
• Serves on-call rotation for home care.

Requirements

• Graduate of an accredited school of nursing.
• One year experience working with the frail or elderly population required.
• Three years RN experience with at least twelve months experience working with the frail or elderly population preferred.
• Experience in home care setting preferred.
• Knowledge and skills necessary to employ the nursing process to perform care for patients.
• Coordinating home care services.
• Assessment of participants within their home environment.
• Serves as nursing liaison to family and participants.
• Serves as a liaison to contracted home care agencies.
• Oversees utilization of home care services at PACE.
• Meets a standardized set of competencies for the specific position description established by PHSC and approved by CMS before working independently.

Qualifications

• A RN currently licensed in NC.
• Presents credentials which demonstrate ability to meet performance criteria for the position.
• Valid driver license and ability to drive to and from the home of participants.
• Medical clearance for communicable diseases and up-to-date immunizations.

Skills

Nursing management, leadership, coordination, assessment, education, and collaboration skills are essential.

Pay

$79,424 - $102,707

Monday through Friday, 8:00am to 5:00pm
