About the role
OCES is expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, and others. As a Home Care Intake Worker, you will evaluate the need for home care services by applicants under the home care program and develop/initiate home care service plans. This is a newly developed program at OCES, which supports older adults and individuals with disabilities by providing vital information and coordination of services to promote healthy, safe living.
Responsibilities
- Conduct intake need assessments and develop consumer-centered care plans with consumers, caregivers, and families.
- Initiate and coordinate services rendered by providers.
- Advocate for consumers in obtaining benefits and services for which they are eligible.
- Create case records in compliance with regulations and OCES standards.
- Make initial home visits for determination of program/service.
- Participate in interdisciplinary activities including with the Transition Support Team.
- Provide back-up support for Information & Referral Specialists performing general intake and providing information and referral to consumers and applicants.
Requirements
- Bachelor’s degree in social work or related field.
- Current Social Work License preferred.
- Minimum one year of experience in the social services field preferred.
- Considerable knowledge of social work as well as the social, emotional, and physical processes of aging.
- Ability to obtain information of a personal nature through interviews, draw appropriate conclusions, and make suitable recommendations.
- Ability to respond appropriately to a variety of emergency situations involving the abuse and/or neglect of elders.
- Excellent written and verbal communication skills.
- Proficient with Microsoft Office Suite or related software.
