About the Role

Rockaway Home Care, a division of Foundation For the Elderly, is seeking a compassionate and organized Home Care Intake Coordinator to join our team in St. Louis, Missouri. As a vital member of our home health services team, you will play a critical role in ensuring a seamless intake process for our clients and their families.

Responsibilities

• Coordinate the intake process for new clients, including patient assessments, insurance verifications, and paperwork completion
• Collaborate with our care team to develop personalized care plans that meet the unique needs of each client
• Communicate effectively with clients, families, and healthcare providers to ensure a smooth transition into our home care services
• Maintain accurate and up-to-date records of client information, care plans, and service delivery
• Identify and address any issues or concerns that may arise during the intake process
• Demonstrate a commitment to our company slogan, "You're not a patient, you're family," by treating each client with dignity, respect, and compassion

Requirements

• High school diploma or equivalent required; degree in a related field preferred
• Minimum of 1 year of experience in a healthcare setting or home care organization
• Strong communication and organizational skills, with the ability to work effectively in a fast-paced environment
• Proficiency in electronic medical records and Microsoft Office
• Familiarity with state and federal regulations governing home care services
• Ability to work independently and as part of a team
