Compensation
- $70/visit for initial assessment
- $60/visit for 60-day visits
Responsibilities
- Setting appointments with Visiting Angels' clients in their Burlington County homes during a regular Visiting Angels' care shift
- Mileage is reimbursed per Company policy
- Create in-home initial client assessments and care plans based on clients' specific care needs
- Conduct follow-up in-home assessments at specified intervals
- Conduct periodic telephone assessments
- Revise care plans as necessary
- Case management
- Supervise Certified Home Health Aides (CHHAs)
- Perform Orientations/Skills Assessments of CHHAs
Qualifications
- Prior Home Care RN experience
- STRONGLY preferred: Current New Jersey Registered Nurse license
- Ideal candidate resides in Burlington County
- Enjoys meeting with and talking with older adults
- Ideal candidate has prior RN experience with in-home care assessments
- Ideal candidate has case management experience
- Excellent written and verbal communicator with strong interpersonal skills
- Must be self-motivated, organized, and able to work independently
- Able and willing to travel to clients' homes throughout Burlington County
- Has a valid driver's license and a reliable car, willing to travel to clients' homes
- Has basic computer skills (data entry on a web-based program, email, scanning)
- Has weekday flexibility
