Home Care Field Registered Nurse RN Burlington County

Visiting Angels is seeking a New Jersey licensed Registered Nurse (RN) for in-home visits to clients throughout Burlington County and occasionally Mercer County (optional). This is a non-medical per diem position with no set hours, offering flexible scheduling ideal for an RN who desires significant work flexibility. Most visits occur on weekdays.

About the role

• Prepare thorough and accurate Initial Assessments and Reassessments
• Develop personalized care plans based on clients' specific care needs

Responsibilities

• Set appointments with clients in their Burlington County homes during regular care shifts (mileage reimbursed per company policy)
• Create in-home initial client assessments and care plans
• Conduct follow-up in-home assessments at specified intervals
• Perform periodic telephone assessments
• Revise care plans as necessary
• Provide case management
• Supervise Certified Home Health Aides (CHHAs)
• Perform Orientations and Skills Assessments of CHHAs

Requirements

• Current New Jersey Registered Nurse license
• Prior Home Care RN experience strongly preferred
• Ideal candidate resides in Burlington County
• Enjoys working with and talking with older adults
• Prior RN experience with in-home care assessments
• Case Management experience
• Excellent written and verbal communication skills with strong interpersonal abilities
• Self-motivated, organized, and able to work independently
• Able and willing to travel to clients' homes throughout Burlington County
• Valid driver's license and reliable car
• Basic computer skills (data entry on a web-based program, email, scanning)
• Weekday flexibility

Pay

$70 per visit for initial assessments and $60 per visit for 60-day visits.
