About the role
Join our team as a Home Care Coordinator! We are seeking an experienced professional to manage and coordinate home care services for our clients.
Responsibilities
- Manage client schedules and ensure timely service delivery
- Coordinate with healthcare providers to provide comprehensive care plans
- Communicate regularly with clients and their families to monitor progress and address concerns
- Ensure compliance with all regulatory standards and policies
Requirements
- Bachelor’s degree in Healthcare Administration, Social Work, or related field
- Minimum 3 years of experience in home health care coordination
- Excellent communication and interpersonal skills
- Proficient in Microsoft Office Suite
Qualifications
- Valid driver’s license and reliable transportation
- Ability to work flexible hours including evenings and weekends
- Strong organizational and time management skills
