About the role
The Home Care Coordinator is responsible for managing a team of caregivers, ensuring patient safety and satisfaction, and coordinating care plans.
Responsibilities
- Manage a team of caregivers to ensure quality care and compliance with standards.
- Develop and implement care plans based on patient needs.
- Monitor patient progress and adjust care plans as necessary.
- Ensure all caregivers are trained and certified in relevant areas.
- Communicate effectively with patients, families, and other healthcare providers.
- Handle emergencies and provide immediate care as needed.
Requirements
- Bachelor’s degree in Healthcare Administration, Nursing, or related field.
- Minimum 3 years of experience in home health care or related field.
- Valid driver’s license and reliable transportation.
- Excellent communication and interpersonal skills.
Qualifications
- Strong organizational and time management skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office Suite.
Skills
- Knowledge of home health care regulations and standards.
- Experience with electronic health records (EHR).
