About the role

The Care Coordinator Supervisor represents the Agency as a frontline customer service liaison between patients/clients, caregivers, and the CSM. They work under the direction of the CSM and coordinate daily operations of payroll and billing processes, customer support, personnel files, and medical records maintenance.

Responsibilities

  • Supports the needs of the patient/client by conferring with the CSM for direction and offering the case to qualified staff.
  • Maintains an active employee list of available caregivers to facilitate prompt assignment of staff to each case.
  • Ensures caregivers meet all Company policy requirements and are willing to work during compatible time frames.
  • Facilitates continuity of care by assigning the same caregiver to each patient/client whenever possible.
  • Schedules patient/client care according to the plan of care, ensuring only requested visits are made.
  • Validates caregiver documentation against schedules and appropriately resolves differences.
  • Updates active patient/client and caregiver lists weekly, or more frequently as requested.
  • Assists the staffing coordinator in meeting staffing needs for clients, considering pay and bill rates and caregiver qualifications.
  • Makes staffing changes in accordance with Company policy when requested by the patient/client or caregiver.
  • Contacts patients and families to confirm satisfaction after new caregivers make their first visit and may conduct random phone calls to assess customer satisfaction.
  • Documents all requests for changes in caregivers, logs complaints, and records results of customer satisfaction calls.
  • Hires caregivers by identifying recruitment needs, screening applicants, administering evaluations, arranging interviews, and verifying references.
  • Provides support to the Client Services Manager in marketing the business, completing market research, and reviewing successful marketing campaigns.
  • Answers the Agency’s phone lines courteously and provides clerical support to the Staffing Coordinator.
  • Handles incoming and outgoing mail, performs payroll processes, validates bills for all payers, supports scheduling patient visits, and audits medical supplies.
  • Maintains personnel files, audits file accuracy, and reports incomplete, outdated, or inaccurate information to the Client Services Manager.
  • Maintains medical supplies inventory, tracks usage, and ensures billing accuracy.
  • Communicates identified discrepancies or problems, prepares documents in response to requests, and records subpoenas.
  • Performs general clerical functions to facilitate home health operations and provides backup support as needed.
  • Audits records to assist management in maintaining compliance and integrity of information.

Qualifications

  • Bachelor's degree in healthcare, human services, or related field.
  • Minimum of 3 years of experience in home health care, patient care, or related field.
  • Knowledge of agency policies and procedures.
  • Excellent communication and interpersonal skills.
  • Ability to manage multiple tasks and prioritize responsibilities.
  • Proficiency in Microsoft Office Suite.
