Qualifications
- Health Care Aide Certificate from a recognized Health Care Aide program
- Other combinations of suitable education and experience may be considered
- Province of Manitoba Class 5 Drivers License, or equivalent from province of residence, and access to a personal vehicle to provide service within Prairie Mountain Health
- Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology
- Demonstrated organizational skills, and the ability to work independently
- Demonstrated problem solving and decision-making skills
- Demonstrated flexibility to facilitate changes in techniques and procedures in a changing environment
- Demonstrated knowledge and competence of skills and concepts related to the position
- Demonstrated communication skills
- Ability to respect and promote confidentiality
- Ability to perform the duties of the position on a regular basis
- Ability to respect and promote a culturally diverse population
- Ability to work effectively and maintain positive working relationships with co-workers, clients and within interdisciplinary team
Responsibilities
- Demonstrates respect for client independence, dignity, privacy, and lifestyle at all times.
- Supports clients’ independence in activities of daily living, as assessed by the Case Coordinator.
- Demonstrates knowledge of standards for safety and sanitation including guidelines for hand washing.
- Reports concerns of knowledge and/or skill deficits to the Supervisor.
- With additional education and in accordance with PPG-00819, Assignment of Medication and Treatment Tasks to Home Care Attendants, may be assigned additional tasks as required.
- Demonstrates ability to effectively utilize electronic programs.
- Performs all communication functions (mail, email, photocopying, faxing, telephone) as required in accordance with Personal Health Information Act (PHIA) and Privacy Act.
- Aids with meal preparation in accordance with the “Canada Food Guide” or the client specific diet.
- Organizes work according to client’s needs and the assigned service request.
- Reports any changes in the client status or home environment to the Supervisor and/or Case Coordinator as per PPG-00738, Observation Reporting- Home Care Program and PPG-02154, Observation Reporting- Supportive Housing Program.
- Reports incidents immediately to the Supervisor or Case Coordinator and/or the client contact and enters in the Incident Reporting System as required.
- Participates in staff meetings, case conferences, and in-service programs as required.
- Communicates with co-workers involved to achieve the client-centered goals.
- Other duties as assigned.
