Responsibilities
- Provide hands-on care with warmth and dignity: Assist participants with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and safely transferring them with care.
- Help with everyday tasks: Support with Instrumental Activities of Daily Living (IADLs) like laundry, light housekeeping, grocery shopping, reminders for appointments, and medication schedules.
- Monitor and promote well-being: Measure and document vital signs, ensuring participants’ health is always our top priority.
- Encourage strength and independence: Assist with personalized home exercise programs, guided by the Interdisciplinary Team (IDT) and thoughtfully designed to help participants thrive.
- Provide a helping hand with transportation: Help participants enter and exit their homes safely, offering support as a medical escort for appointments when needed.
- Be their advocate: Observe and report any changes in condition or behavior to the supervisor, ensuring participants receive the right care at the right time.
- Keep care seamless: Maintain accurate, up-to-date records, including daily logs and progress notes in the electronic medical record system.
- Foster connection and understanding: Communicate with participants, their families, colleagues, and partner agencies with clarity and compassion.
- Champion respect and privacy: Protect participant confidentiality and treat them with the dignity they deserve.
- Offer companionship and joy: Build meaningful connections by engaging in conversation, providing emotional support, and creating a positive, uplifting environment for every participant.
Qualifications and Experience
- A heart for service: Passionate about supporting high-risk seniors and older adults, ensuring they feel safe, respected, and valued.
- Experience working with older adults: Previous experience with the frail and elderly population is a plus.
- CPR and First Aid certification: If you don't have these yet, no worries! We'll provide training, and you’ll have 60 days from hire to complete it.
- CNA or HHA certification preferred: These credentials help, but if you don't have them, we’re happy to assist you in obtaining the right training.
- Flexibility: Willingness to support the team by working at the PACE center when needed.
- Bilingual abilities: We welcome candidates who can communicate with participants in multiple languages!
- Valid CA driver’s license and reliable transportation: A clean driving record and auto insurance (as required by state law) will ensure you’re ready to assist with transportation needs.
- Relevant caregiving experience: Previous work as a personal care attendant, CNA, or in a similar caregiving role is highly valued.
- Training and registration support: If you haven’t completed a State-approved Nurse Aide, HHA, or Personal Care Assistant course, we’ll help you get started! Plus, we’ll guide you through the Home Care Registry process before your first day.
