Home and Community-Based Services Assessor (Roswell/SE NM)
About the role
Join our team as a Home and Community-Based Services (HCBS) Assessor. Conduct annual in-home assessments across the southeast New Mexico region with adults receiving services through the New Mexico Mi Via Waiver. Manage a flexible, primarily field-based schedule visiting participants' homes, completing assessments, and using your home office for follow-up, scheduling, and documentation.
Responsibilities
- Build meaningful rapport with participants and their families or caregivers during in-home visits.
- Administer a structured assessment tool using electronic systems to gather information on daily living skills, behaviors, and support needs.
- Translate observations and information into accurate, clear documentation that ensures individuals maintain access to community-based services.
- Coordinate and schedule annual in-home assessments, including outreach and follow-up calls, confirming appointments, and maintaining an organized calendar.
- Navigate and use multiple technology platforms to document findings, complete templates, upload assessments, and accurately close out each case in a timely manner.
- Ensure compliance with confidentiality and HIPAA requirements, safeguarding participant information and following established policies and procedures for handling sensitive data.
- Communicate with supervisors, program coordinators, and assessor colleagues to report on your work and discuss cases.
- Participate in training and team meetings.
Requirements
- Associates degree in social work, psychology, other behavioral health science field.
- 2 years of social service, nursing or other relevant work experience.
- Valid, unrestricted driver license, proof of auto insurance, and reliable transportation.
Qualifications
- Bilingual skills, especially Spanish (optional).
- Experience working with adults with disabilities or individuals receiving supportive services (optional).
- Background in healthcare or human services settings (optional).
Skills
- Strong oral and written communication and documentation skills.
- Proficiency with computers and software tools, including the ability to learn new systems.
- Comfort conducting in-home visits with reliable personal transportation.
- Strong time management and organizational skills.
- High level of reliability, accountability, and team collaboration.
