Home and Community Based Services Assessor (Las Cruces)

About the role

In this role, you’ll conduct annual in-home assessments across Las Cruces and the Southern New Mexico region with adults receiving services through the New Mexico Mi Via Waiver. You’ll manage a flexible, primarily field-based schedule—visiting participants’ homes, completing assessments, and using your home office for follow-up, scheduling, and documentation.

Responsibilities

  • Build meaningful rapport with participants and their families or caregivers during each visit.
  • Administer a structured assessment tool using our electronic systems.
  • Gather detailed information about daily living skills, behaviors, and support needs.
  • Translate observations and information into accurate, clear documentation.
  • Ensure compliance with confidentiality and HIPAA requirements.
  • Communicate with your supervisor, program coordinator, and assessor colleagues to report on your work and discuss cases.
  • Participate in training and team meetings.

Requirements

  • Associate's degree in social work, psychology, or other behavioral health science field.
  • 2 years of social service, case management or other relevant work experience.
  • Valid, unrestricted driver license, proof of auto insurance and reliable transportation.
  • Ability to pass a Motor Vehicle Record check.

Qualifications

  • Strong oral and written communication and documentation skills.
  • Proficiency with computers and software tools, including the ability to learn new systems.
  • Comfort conducting in-home visits with reliable personal transportation.
  • Strong time management and organizational skills.
  • High level of reliability, accountability, and team collaboration.
  • Comfort working with individuals with complex needs.

Skills

  • Bilingual skills, especially Spanish (optional).
  • Experience working with adults with disabilities or individuals receiving supportive services (optional).
  • Background in healthcare or human services settings (optional).
