HOC Case Manager

Mission-driven role assisting individuals enrolled in HOC’s Supportive Services program in obtaining and maintaining permanent, stable housing.

Responsibilities

  • Develop client-centered housing stability plans and work closely with individuals to facilitate movement into permanent, stable housing using housing-first principles and progressive engagement.
  • Complete relevant assessments according to funder and contractual guidelines.
  • Facilitate connections to community resources, including landlord vacancies, and work directly with clients to mitigate evictions, poor credit, and criminal histories.
  • Connect clients with vocational services, SSI/SSDI, and public benefits where applicable.
  • Fill in for respite staff as needed to support client care and program operations.
  • Provide FCS case management support, including assessments, service coordination, referrals, documentation, and billing.
  • Support the RC Program by enrolling clients and assisting them in achieving employment readiness goals through skills assessment, mentoring, job search support, resume and interview preparation, and referrals to vocational training and employment opportunities.
  • Maintain consistent client engagement through active outreach, progressive engagement strategies, and meeting required client engagement expectations.
  • Maintain legible and complete client files according to funder requirements and ensure real-time, accurate data entry in HMIS.
  • Complete monthly reviews of project-relevant data and provide updates to direct supervisor.
  • Seek continuous improvement in the quality of HOC operations and case management programs.
  • Maintain professional boundaries with clients and co-workers and adhere to an appropriate self-care plan.
  • Stay current with best practices such as progressive engagement, motivational interviewing, mental health first aid, and harm reduction.
  • Ensure confidentiality, integrity, and security of client protected health information (PHI) per HIPAA regulations.
  • Utilize all forms of communication (email, text, phone) and database practices as directed by supervisor.
  • Act as a mandated reporter, following agency policies to report suspected or confirmed abuse or neglect of children or adults.
  • Adhere to the tenants of Catholic Social Teaching and Catholic Doctrine.
  • Perform related functions necessary to support the mission and core values of Catholic Charities.

Requirements

  • Bachelor’s degree in social services or related field with a minimum of one year of relevant experience in direct client services; OR an AA Degree with three years of relevant experience in direct client services.
  • Professional experience in low-barrier homeless services or mental health services preferred.
  • Valid Driver’s License and successful passing of a background check applicable to the position.

  • Language Ability: Ability to read, analyze, and interpret technical procedures and funder regulations; write reports; and present information effectively.
  • Mathematical Ability: Ability to perform basic arithmetic operations.
  • Motor Coordination: Ability to coordinate eyes, hands, fingers, and feet accurately for precise movements.
  • Problem-Solving Ability: Ability to identify and prevent problems, formulate alternative solutions, and transfer learning from past experiences.
  • Quality Management: Ability to complete duties with precision, edit work for accuracy, apply feedback, and generate ideas to improve quality.
  • Reasoning Ability: Ability to solve practical problems and interpret instructions in various formats.

Physical Abilities

  • Regularly sit, stand, climb, walk, hear/listen, and talk.
  • Frequently lift up to 25 pounds, pull/push, carry, grasp, and reach.
  • Occasionally crawl, stoop, and kneel.
  • Ability to see clearly at 20+ feet, with or without corrective lenses, and focus.

Workplace Environmental Conditions

  • Exposure to moderate to loud noise levels during shifts.
  • Subject to high and low temperatures.
  • Exposure to strong smells such as urine or body odor.
  • Client population may include individuals involved in gangs, with substance use concerns, or experiencing homelessness.
