About the role
PHMC serves as both a direct service provider to individuals, families, and communities across the region and an intermediary agent managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives. The HIV Care Manager will oversee performance, staffing, and funding for HIV Care across PHMC Health, ensuring operational sophistication, strategic leadership, and deep mission alignment.
Responsibilities
- Oversee performance, staffing, and funding for HIV Care across PHMC Health.
- Serve as a liaison to the City, State, and other grant funders regarding all HIV-treatment and prevention activities.
- Manage all grants related to HIV treatment and prevention, including compiling written reports and facilitating health center quality reviews and site visits.
- Champion and educate health center staff on all Ryan White funding requirements.
- Collaborate with all health center disciplines to ensure patients’ appropriate and continuous access to Ryan White services.
- Convene regular, interdisciplinary health center meetings to coordinate care and review performance outcomes.
- Lead process improvement efforts and serve as a spokesperson with external funders.
- Work with PHMC Leadership and Finance teams to ensure optimal use of program resources.
- Ensure social service staff are trained on programs for individuals living with HIV and on HIV prevention services.
- Maintain regular, ongoing supervision of social service staff working with HIV+ patients or those at risk for HIV.
- Collaborate with the Medical Director and Director of Behavioral Health and Social Services to ensure appropriate documentation of Ryan White data and staff training.
- Serve as a liaison between PHMC Health and health center disciplines, City, State, nonprofit funders, pharmaceutical benefits programs, and local community organizations.
- Ensure compliance with DHH and HRSA standards related to HIV treatment, case management, testing, and PrEP care coordination.
- Monitor, analyze, and communicate performance data related to Ryan White measures.
- Provide weekly supervision to HIV Medical Case Managers and PrEP Patient Navigators.
- Ensure accurate and timely documentation of client encounters, forms, and applications in the EMR and CAREWare.
- Conduct bi-weekly chart audits in the EMR and CAREWare to ensure documentation quality and accuracy.
- Maintain up-to-date knowledge of community resources and public benefits.
- Provide trauma-informed therapeutic support in collaboration with an interdisciplinary team.
- Keep abreast of federal, state, and city regulations, laws, and policies as they exist and change.
- Comply with HIPAA, DPH, The Joint Commission, and other accreditation and regulatory standards.
- Adhere to all PHMC policies and procedures.
Requirements
- 2 years of case management and/or care coordination experience required.
- Master’s degree from an accredited university or Bachelor’s degree in social work, psychology, sociology, or related field with 3 years of infectious diseases, HIV medical case management, PrEP, and/or care coordination experience required.
- Exceptional knowledge of Ryan White and/or Philadelphia Division of HIV Health (DHH) services.
