Clinical Care Manager responsible for coordinating home health and hospice services to deliver safe, compassionate, and high-quality care in patients' homes.

Responsibilities

• Manage a caseload of patients, guiding field clinicians and collaborating with physicians and interdisciplinary teams to develop and modify care plans.
• Monitor patient outcomes, ensure regulatory compliance, and participate in case conferences and quality improvement initiatives.
• Support a patient-centered approach with a focus on teamwork, integrity, and work–life balance.
