Responsibilities
- Scan and attach referral appointment letters and consultation reports to the appropriate electronic clinical record, including creating an order if needed and routing to the provider (if not auto-routed) for review within 48 hours of receipt.
- Scan and attach all transferred records, requested records, and other “paper” records (EKGs, fetal stress tests, other correspondence) to the appropriate electronic clinical record.
- Monitor electronic fax folders (Provider Flow) and process received faxes, including attaching documents to patient electronic clinical records and routing to providers/care teams for review; appropriately forward other faxes received (e.g., medication refill requests).
- Handle incoming telephone calls regarding medical records needs from CommWell Health sites, outside organizations, and patients.
- Process medical record requests/releases, including reviewing requests, producing copies (paper or CD), faxing or mailing records, preparing records charges (if applicable), and documenting the request.
- Process medical/behavioral health record requests/releases, including review, copying, faxing or mailing, charge preparation, and documentation.
- Process behavioral health record requests/releases, including review, copying, presenting to assigned counselor for final approval, packaging for mailing, charge preparation, and documentation.
- Retrieve patient records from outside agency portals for hospitalization and/or specialty visits.
- Process subpoenas for records, including reviewing with supervisor, producing copies, documenting releases, and packaging for return receipt mailing or hand delivery if required.
- Manage duplicate EPIC records, including resolving pending issues directly or via EPIC help desk, verifying information to confirm duplication, and merging records as needed.
- Assist Referral Specialist in closing the “referral loop” by obtaining delinquent referral consult reports.
- Assist with management of stored paper records, including moving shelving and processing records for destruction.
- Monitor “overnight” voicemail box, receive messages, and task or email messages to appropriate recipients.
- Assist approved outside record auditors (e.g., Medicaid, CDC) with obtaining necessary information from the electronic health record.
- Assist VP/Chief of Environment of Care Quality with medical records audits and clerical needs.
Requirements
- High School Diploma or GED.
- Clean driving record with current NC driver’s license, approved by corporate vehicle insurance vendor for driving privileges.
