About the Role

Responsibility for processing patient medical records in accordance with regulatory and accreditation standards.

Responsibilities

  • Retrieve patient medical records from nursing units upon discharge.
  • Reconcile, prep, scan, index, and perform quality control per procedure.
  • Process requests for Protected Health Information (PHI) from patients, hospitals, physician offices, insurance companies, and auditors while following confidentiality rules, State, and Federal laws.
  • Validate patient authorization forms and collaborate with various departments to gather complete legal medical records.
  • Process subpoenas per protocol.
  • Process MyChart activation codes.
  • Facilitate Release of Information requests using vendor software.
  • Facilitate paper storage requests with the storage vendor.
  • Answer and respond to phone calls accordingly.
  • Analyze electronic health records for documentation deficiencies.
  • Communicate with physicians regarding incomplete or delinquent medical records.
  • Provide training and assistance to physicians on the use of the electronic medical record.
  • Monitor transcription queues, deficiency error queues, scan indexing queues, and perform document troubleshooting and corrections.
  • Review medical records for content, completion timeliness, and compile results for physician suspension; report information to administrators.
  • Collect information for the completion of birth certificates.
  • Accurately and timely enter and upload birth certificate and birth record information to the State of Michigan.

Requirements

  • High School Diploma or equivalent (Required).
  • 2 years of relevant experience.
  • 2 - 4 years of office or healthcare experience.

Qualifications

  • Associate's degree (Preferred).

Schedule

  • Full time, evening shift (3:30 p.m. - 12:00 a.m.).
  • 40 weekly scheduled hours.
  • Days worked: Monday - Friday.
  • Weekend frequency: Every fourth weekend.

Primary Location: Royal Oak Hospital - 3601 W 13 Mile Rd, Royal Oak, Department: HIM Operations CHE - Corporate.
