Necessary Documents:
- Original HHA/PCA Certificate.
- Identification card, citizenship, residence or work permit.
- Social security (original).
- Two professional reference letters.
- Physical Statement (must be dated within 9 months of hire), with habituation statement.
- Negative PPD Information Step 1 and Step 2 (within 1-3 weeks from each other) OR Whole Assay Blood Test.
- Positive PPD (needs the following):
- A) Written documentation by Doctor with history of Positive PPD.
- B) Negative Chest X-Ray Report.
- Rubella Immunity Proof (lab printed report).
- Rubeola Immunity Proof (lab printed report).
- Drug Screen (lab printed report).
- Proof of Flu Vaccine for the season (if you did get it).
- Direct Deposit form, Voided check or Routing & Bank account numbers.
