About the role

Connected Home Care is looking to further expand our team and bring on dedicated, reliable Home Health Aides (HHA) & Personal Care Aides (PCA).

Responsibilities

• Assists patients by providing personal services, such as bathing, dressing, and grooming
• Social interaction
• Report any changes in the patient’s condition or incidents to RN Supervisor or therapist

Requirements

• Valid (HHA) Home Health Aide OR (PCA) Personal Care Aide Certificate
• Current Photo ID & Social Security card
• Drug screen (last 6 months)
• Complete physical (within the last 6 months)
• 2 reference letters (Personal or Professional)
