Community Home Health Aide

The Community Home Health Aide position at Midland Care Connection focuses on assisting older adults and individuals with chronic illness or mobility/cognitive impairments in maintaining their independence and comfort in their own homes.

Responsibilities

  • Assist clients with daily personal care needs such as bathing, dressing, grooming, using the restroom, eating, and moving around the home.
  • Provide companionship and emotional support to clients.
  • Help with light housekeeping, laundry, meal preparation, and medication reminders.
  • Follow a care plan created by nurses and other healthcare professionals.
  • Report any changes in a client’s condition to nursing staff or supervisors.
  • Perform job duties that may include transferring clients from beds or chairs, assisting with walking, and traveling between clients' homes.
  • Work independently in clients' homes while following care plans.

Qualifications

  • Current Home Health Aide (HHA) certification in the state of Kansas.
  • High school diploma or GED.
  • Ability to provide hands-on assistance with activities of daily living (ADLs).
  • Basic knowledge of infection control, safety practices, and patient rights.
  • Observation, documentation, and reporting skills regarding client conditions.
  • A compassionate, respectful, and patient-centered approach to care.
  • Strong communication and interpersonal skills.
  • Physical ability to perform job duties, including lifting, bending, and standing for extended periods.
  • Valid driver's license, reliable transportation, and willingness to travel to clients' homes.
  • Pass background checks, health screenings, and required trainings.

Preferred Qualifications

  • Previous experience working with older adults, individuals with chronic illness, or those with mobility or cognitive impairments.
  • Home health, hospice, palliative care, or PACE program experience.
  • Experience assisting with light housekeeping, meal preparation, and medication reminders.
  • Familiarity with interdisciplinary care teams and community-based healthcare models.
  • CPR/First Aid certification.
  • Demonstrated ability to provide emotional support and companionship to clients.

Pay and Benefits

  • Pay Rate: $20.50+/hour
  • Shift Differentials: $1.00 Weekdays | 5:00pm – 8:00pm, $1.50 Weekends | 8:00am – 5:00pm, $2.00 Weekends | 5:00pm – 8:00am
