HHA - Home Health Aide

Job Responsibilities

• Perform home care visits and work under the supervision of a Registered Nurse or Therapist.
• Provide various personal care services as necessary to meet the patient’s needs.
• Observe, report, and document observations and patient care performed during the home care visit.
• Prepare light meals and assist in feeding the patient if necessary.
• Ambulate and exercise the patient according to the plan of care.
• Perform range of motion and other simple procedures as an extensional therapy service as ordered.
• Absorb and administer medication as directed.
• Assist with the patient’s laundry if needed.
• Meet safety needs of patients and use equipment safely and properly (foot stools, side rails, etc.).
• Report patient’s condition and significant changes to the assigned nurse.
• Adhere to the Organization's documentation and care procedures and standards of personal and professional conduct.
• Maintain the confidentiality of protected healthcare information (PHI).

Job Requirements

• Tact, patience, and empathy for the needs of the ill, injured, frail, and the impaired are essential.
• Previous experience is great but not required.
• Has the ability to read, follow written instructions, and document services provided.
• Self-directed with the ability to work with little direct supervision.
• Kind, reliable, and compassionate.

Qualifications

• Current CPR certification or obtain one upon hire.
• 18 years of age or older.
• Meets the training requirements in accordance with State and Federal laws (minimum 40 hours training).
• One year of hands-on work experience, preferred (can include experience in a home care, hospital, or assisted living setting).
• Ability to read and follow written instructions and document care given.
• Self-directing with the ability to work with little direct supervision.
• Current CPR certification.
