Job Responsibilities
- Perform home care visits and work under the supervision of a Registered Nurse or Therapist.
- Provide various personal care services as necessary to meet the patient’s needs.
- Observe, report, and document observations and patient care performed during the home care visit.
- Prepare light meals and assist in feeding the patient if necessary.
- Ambulate and exercise the patient according to the plan of care.
- Perform range of motion and other simple procedures as an extensional therapy service as ordered.
- Absorb and administer medication as directed.
- Assist with the patient’s laundry if needed.
- Meet safety needs of patients and use equipment safely and properly (foot stools, side rails, etc.).
- Report patient’s condition and significant changes to the assigned nurse.
- Adhere to the Organization's documentation and care procedures and standards of personal and professional conduct.
- Maintain the confidentiality of protected healthcare information (PHI).
Job Requirements
- Tact, patience, and empathy for the needs of the ill, injured, frail, and the impaired are essential.
- Previous experience is great but not required.
- Has the ability to read, follow written instructions, and document services provided.
- Self-directed with the ability to work with little direct supervision.
- Kind, reliable, and compassionate.
Qualifications
- Current CPR certification or obtain one upon hire.
- 18 years of age or older.
- Meets the training requirements in accordance with State and Federal laws (minimum 40 hours training).
- One year of hands-on work experience, preferred (can include experience in a home care, hospital, or assisted living setting).
- Ability to read and follow written instructions and document care given.
- Self-directing with the ability to work with little direct supervision.
- Current CPR certification.
