HEDIS Stars/Quality Coordinator /Population Health
Optima Medical is an Arizona-based medical group with 30+ locations and over 130 providers, serving more than 200,000 patients statewide. We offer a full spectrum of services including cardiovascular health, behavioral health, allergy testing, in-house lab testing, imaging, chronic disease management, and other specialty services.
About the role
This role reports to the Manager of Population Health and supports value-based care and population health initiatives. The coordinator will analyze data, collect records to close HEDIS Quality Measures, participate in health plan performance meetings, and support related efforts. The position requires full-time in-office presence.
Responsibilities
- Collect data and patient records to close HEDIS care gaps and maintain STAR Ratings
- Meet with multiple health plan representatives regarding incentive programs
- Work within Provider Portals to close care gaps
- Attest to patient's historical diagnosed chronic and suspect conditions
- Assist Manager with participation in population health programs
Requirements
- 1 year of experience in Quality Care Gaps / HEDIS Stars (required)
- Experience in a corporate or healthcare setting
- Medical Assistant (MA) experience highly preferred but not required
- High school diploma or GED equivalent
