Heart Failure Team, NP/PA

About the role

The selected candidate will be responsible for providing clinical leadership and administrative oversight for our team of APCs across multiple sites, including Christiana and Wilmington. With more than 2,000 primary heart failure discharges per year, you will have every opportunity to make a significant impact on the outcomes of our heart failure population.

Responsibilities

• Maintain on-call coverage in a rotating call schedule with the other 4 APC’s for the LVAD program (must be within a 45 minute response time of the hospital while on call).
• Utilize cutting edge technology and health informatics to assist clinical skills in providing outstanding care to a wide array of patients.
• Participate in innovative programs to enhance compliance/optimization of Guideline Directed Medical Therapy (GDMT) along with prevention of unnecessary early readmissions.
• Provide clinical leadership and administrative oversight for our team of APCs across multiple sites, including Christiana and Wilmington.
• Be part of the care team for the LVAD program, from initial evaluation through post-op management.

Requirements

• Master’s Degree in Nursing (nurse practitioner) or Master’s Degree in Physician Assistant studies.
• Acute Care Nurse Practitioner certification required.
• Experience preferably in cardiology or advanced heart failure.
• Prior leadership or supervisory experience strongly preferred.
• ACLS required.

Qualifications

• Licensure or license eligible in the state of Delaware.

• Knowledge of health informatics and technology.

Pay

Annual Compensation Range: $115,474.00 - $143,579.00
