Scope
Nurse Practitioners (APRN) / Physician Assistants provide advanced heart failure consultations within the Skilled Nursing Facility setting throughout the East Region as well as scheduled time in the advanced heart failure outpatient clinic located in Norwich, CT.
Responsibilities
- Assess newly admitted heart failure patients to the SNF setting
- Collaborate with other providers on Cardiac Care with the goal of reducing hospital readmissions
- Aid in coordinating discharge providing bridge appointments in the outpatient clinic
Requirements
- Graduate of an accredited Physician Assistant or Nurse Practitioner program
- Licensed, or eligible to be licensed, as a Physician Assistant, or Advanced Practice Registered Nurse (APRN) in the state of Connecticut
- Certified or eligible to be certified as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse
- Cardiology experience required
- Geriatric board certification preferred
