About the Role

The Division of Cardiology at Albany Medical College is seeking a full-time Nurse Practitioner or Physician Assistant to join their specialized Heart Failure outpatient practice. This role offers the opportunity to provide high-quality, patient-centered care in a collaborative environment while working closely with heart failure cardiologists and a multidisciplinary care team.

Responsibilities

• Conduct outpatient visits for patients with heart failure, including ongoing management and follow-up care
• Perform comprehensive patient assessments, including detailed medical histories and physical examinations
• Order and interpret diagnostic studies such as laboratory tests and cardiac imaging
• Develop and implement treatment plans in collaboration with supervising physicians
• Provide medication management, patient education, and counseling on disease management and lifestyle modifications
• Care for coordination of care with primary care providers and other specialists
• Respond to patient inquiries related to ongoing care
• Document patient encounters, treatment plans, and coding accurately in the electronic health record
• Support clinic workflow and patient throughput as part of a team-based care model

Qualifications

• Current license and active registration as a Nurse Practitioner or Physician Assistant in New York State
• Master’s degree from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program
• Interest in or experience with cardiovascular or heart failure care preferred
• Familiarity with electronic medical records; Epic experience preferred
• Strong communication and collaboration skills
• Commitment to patient-centered care and maintaining HIPAA confidentiality standards
