About the role

The HF CNS Service operates Monday to Friday during business hours. However, a new model of a 7-day working week may be implemented in the future. We provide holistic and evidenced-based specialist care to people diagnosed with HF across NHS Fife. We have two bases at Stratheden Hospital and Lynebank Hospital.

Responsibilities

• Participate in the ongoing development, implementation, and evaluation of the HF clinical service
• Improve the management, treatment, and support of patients with HF, and their carers/families
• Manage your own caseload
• Contribute to ongoing service development
• Play a key role in preventing avoidable hospital admissions through proactive, patient-centered care

Requirements

• Experienced HF nurse specialist with a non-medical prescribing qualification
• Undertaken Level 11 postgraduate qualifications in clinical assessment & decision-making and chronic heart failure
• Band 7 and senior member of the team, responsible for supporting the Team Leader under the leadership pillar and deputizing for the Team Leader in their absence
• Previous leadership experience is desirable but not essential, as training will be provided

Qualifications

• Experience in cardiology and education to degree level
• Evidence of recent CPD
• Willingness to undertake Level 11 postgraduate qualifications in clinical assessment & decision-making and chronic heart failure

Skills

• Strong clinical judgement
• Excellent communication skills
• Passion for improving outcomes for people with long-term cardiac conditions
