Healthy First Steps RN Case Manager - Jacksonville and surrounding areas, FL
About the role
At UnitedHealthcare, you'll wear many hats and work in various environments. Your responsibilities include making outbound calls to assess members' health status, taking incoming calls, identifying gaps or barriers in treatment plans, providing patient education, interacting with Medical Directors on challenging cases, coordinating care, making referrals, and coordinating services as needed.
Responsibilities
- Making outbound calls to assess members' current health status (potential for field visits as well)
- Taking incoming calls from members
- Identifying gaps or barriers in treatment plans
- Providing patient education to assist with self-management
- Interacting with Medical Directors on challenging cases
- Cooking care for members
- Making referrals to outside sources
- Cooking services as needed (home health, DME, etc.)
- Educating members on disease processes
- Encouraging members to make healthy lifestyle changes
- Documenting and tracking findings
- Making 'welcome home' calls to ensure that discharged member receive the necessary services and resources
Requirements
- A current and unrestricted Registered Nurse license in the state of Florida
- 3+ years of experience in a hospital setting, acute care, direct care experience within the OB and/or Maternity field
- 1+ years of experience working with MS Word, Teams, Excel and Outlook
- Reside in the Jacksonville metro area
Preferred Qualifications
- A Bachelor's Degree
- Case management experience
- Background in managing populations with complex medical or behavioral needs
- Experience with electronic charting
- Bilingual - Spanish
- Experience with arranging community resources
- Field-based work experience
- Knowledge of Florida Community Maternal Child Resources

Pay and Benefits
- Hourly pay for this role will range from $28.94 to $51.63 per hour based on full-time employment.
