Healthcare Social Worker

Role: Social Worker in a skilled nursing and rehab facility. Key Responsibilities Conduct psychosocial assessments and develop individualized care plans.Coordinate safe discharges and provide counseling, crisis intervention, and advocacy.Collaborate with nursing, rehab, and medical teams to support resident dignity, adjustment, and quality of life.Lead care conferences and connect residents with community resources.Maintain regulatory documentation and contribute to a respectful, resident-centered, team-based environment.
