Healthcare Provider (Full-Time/Part-Time)

About the role

Join a collaborative healthcare team focused on senior patient care as a full-time Primary Care Nurse Practitioner (NP) or Physician Assistant (PA). This role emphasizes value-based, proactive, and patient-centered care that addresses the physical, emotional, social, and behavioral health needs of seniors. You will work on-site at the clinic location.

Responsibilities

• Provide comprehensive, high-quality primary care to senior patients.
• Manage referrals and coordinate care with hospitals, skilled nursing facilities (SNFs), durable medical equipment (DME) providers, and home health services.
• Document care thoroughly and accurately.
• Participate in on-call rotations.
• Adhere to Medicare guidelines and HEDIS indicators.

Requirements

• Master’s degree in nursing (for NP) or equivalent PA education.
• Active, unrestricted Nurse Practitioner or Physician Assistant license in Florida.
• Experience in primary care or Medicare Advantage care.
• Active DEA license.
• Familiarity with Medicare guidelines and HEDIS indicators.
