Healthcare Provider (Full-Time/Part-Time)

About the role

We are seeking a full-time Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join a collaborative healthcare team focused on delivering high-quality, personalized care to seniors.

Responsibilities

• Provide comprehensive senior care following clinical standards
• Collaborate with a diverse care team to ensure coordinated services
• Manage referrals, hospital/SNF coordination, DME, and home health
• Document care with quality-based support
• Participate in on-call rotations and support practice growth
• Experience with value-based care is preferred

Qualifications

• Master's degree in nursing with board certification
• Active, unrestricted NP or PA license in Florida
• Experience in primary care, Medicare Advantage, or value-based care
• Familiarity with Medicare guidelines and HEDIS indicators

Pay

Compensation ranges from $123,800 to $170,300 annually, with eligibility for bonuses.
