Healthcare Provider (Full-Time/Part-Time)

We are seeking a full-time Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join a collaborative healthcare team focused on delivering high-quality, personalized care to seniors. In this role, you will provide comprehensive senior care following clinical standardscollaborate with a diverse care team to ensure coordinated servicesmanage referrals, hospital/SNF coordination, DME, and home healthdocument care with quality-based supportparticipate in on-call rotations and support practice growthexperience with value-based care is preferred Qualifications include Master's degree in nursing with board certificationactive, unrestricted NP or PA license in Floridaexperience in primary care, Medicare Advantage, or value-based carefamiliarity with Medicare guidelines and HEDIS indicators Compensation ranges from $123,800 to $170,300 annually, with eligibility for bonuses.
