About the Role
Supports FMCNA’s mission, vision, core values, and customer service philosophy. Adheres to the FMCNA Compliance Program, including following all regulatory and FMS policy requirements. Ensures provision of quality patient care while maintaining cost-effective clinical operations in accordance with all legal, compliance, and regulatory requirements. As the clinical leader, has authority to make daily decisions to ensure continuity of care, patient safety, and staff safety.
Responsibilities
- Manages the tactical execution of short- and long-term objectives through coordination of activities, including costs, methods, and staffing.
- Provides leadership, coaching, and development plans for all direct reports to maintain an engaged and productive workforce; partners with Human Resources on employee matters.
- Works on issues of diverse scope requiring analysis of data and understanding of current business trends.
- Supports and drives FMS quality standards by meeting all ESRD regulatory requirements and practicing Quality Assessment and Improvement (QAI), including use of FMS QAI tools.
- Addresses and acts on adverse events and action thresholds.
- Oversees facility’s Home Therapies Program if applicable.
- Ensures compliance with all applicable federal, state, and local laws and regulations.
- Implements and communicates all FMS Clinical Quality policies and procedures to facility staff.
- Maintains integrity of medical records and other FMS administrative and operational records.
- Complies with all data collection and auditing activities.
- Maintains facility environmental integrity, including safety.
- Receives assignments in the form of objectives and determines resource use to meet schedules and goals.
- Interprets and recommends changes to policies and establishes procedures impacting the organization.
- Frequently interacts with subordinate supervisors, customers, and functional peer groups on matters involving multiple areas.
- Leads cooperative efforts among project teams and presents at internal and external meetings.
- Acts as an advisor to subordinates to resolve technical problems and meet schedules.
- Responsible for hiring, coaching, counseling, performance reviews, disciplinary actions, and terminations.
- Provides technical guidance to staff.
Patient Care
- Coordinates all aspects of patient care from admission through discharge.
- Ensures patient education on quality measures, transplant options, modality awareness, access care, catheter reduction, and treatment adherence.
- Acts as a resource for patient concerns and questions.
- Accountable for timely completion of patient care assessments and care plans by organizing Interdisciplinary Team meetings.
- Directs efficient and timely patient scheduling to maximize facility station efficiency.
- Develops mechanisms to track patient status, including hospitalizations, no-shows, catheter use, and significant changes in care.
- Develops action plans for unexcused and missed treatments in collaboration with the Director.
- Plans, coordinates, and acts as liaison for patient care per disease management agreements, including eligibility validation and coordination with the multidisciplinary renal team.
- Facilitates timely workup for access management, dialysis services, patient education, hospitalizations, and kidney transplantation.
- Communicates changes in patient status (clinical, insurance, travel, transportation) to the disease management company.
- Maintains current knowledge of disease management software and systems.
Staff Management
- Implements FMS staffing and medical supply models to provide quality patient care and makes recommendations.
- Participates in recruitment, interviews, and hiring decisions for new personnel.
- Ensures completion of new hire orientation, training, and mandatory in-services, including ICD code training.
- Ensures documentation for annual in-service training and policy updates.
- Oversees training to ensure compliance with FMS risk management initiatives.
- Provides opportunities for professional growth and training to ensure clinical competence.
- Oversees performance of licensed personnel, direct patient care staff, and technical staff.
- Provides employee education, guidance, and feedback related to performance.
- Maintains knowledge of FMCNA benefits, HR policies, and procedures; acts as a resource to staff.
- Provides counseling and support to clinical staff at regular intervals.
- Completes timely employee evaluations and establishes annual goals.
- Documents disciplinary conferences and confers with Director and HR on disciplinary decisions.
- Participates in employee recognition and satisfaction programs.
- Creates and communicates efficient and timely employee schedules.
- Implements a Continuous Quality Improvement (CQI) Process Improvement Team involving staff in problem-solving.
Physician Relations
- Facilitates the application process for physician privileges and compliance with FMS Medical Staff By-Laws.
- Builds strong relationships with Directors and physicians; facilitates staff relationships with physicians.
- Ensures regular and effective communication with physicians through meetings.
- Participates in the Governing Body.
- Schedules and coordinates CQI meetings with physicians.
Maintenance and Technical
- Responsible for the integrity and safety of the facility water system.
- Knowledgeable in the operation of all facility equipment and technology.
Administrative Duties
- Maintains and updates all FMS manuals.
- Completes the Annual Standing Order Review and ICD coding.
- Checks and responds to correspondence (electronic, paper, or voice mail).
- Directs information gathering to support billing and collection activities.
- Ensures efficient utilization of medication, laboratory, inventory, supplies, and equipment to meet supply cost goals.
- Participates in the completion and interpretation of the PI-17 inventory supply use analysis.
- Reviews and approves facility payroll.
- Reviews profit and loss statements with the Director.
- Participates in all required Network reporting and on-site state or federal surveys.
- Participates in the completion of the FMS Administrative Clinical Review.
Physical Demands and Working Conditions
- Day-to-day work includes desk and personal computer work, interaction with patients, facility staff, and physicians.
- May require travel between assigned facilities and various community locations, including regional, Business Unit, and Corporate meetings.
- Work environment is characteristic of a healthcare facility with air temperature control and moderate noise levels.
- May be exposed to infectious and contagious diseases/materials.
- Employees are required to take the Ishihara’s Color Blindness test as a condition of employment. Failing the test does not preclude employment; reasonable accommodations will be considered.
Supervision
Responsible for the direct supervision of various levels of staff as designated by region.
Requirements
- Bachelor’s Degree; Advanced Degree desirable or an equivalent combination of education and experience.
- Graduate of an accredited School of Nursing (RN).
- Current appropriate state licensure.
Experience and Skills
- 6–8 years of related experience or an equivalent combination of education and experience.
- 3+ years of supervisory or project/program management experience preferred.
- Minimum of 12 months of clinical nursing experience required; med/surg or ICU/CCU experience preferred.
- Minimum of 6 months of chronic or acute dialysis nursing experience required.
- Must be available as a full-time employee and provide on-call coverage when necessary.
- Demonstrated leadership competencies, including communication, customer service, continuous quality improvement, relationship development, results orientation, team building, motivating employees, performance management, and decision-making.
- Demonstrated management skills to provide leadership in supervision and ensure delivery of high-quality care.
- Must complete Clinical Manager training modules and ongoing developmental programs within the specified timeline.
