Responsibilities
- Fills prescriptions and patient orders, resolves discrepancies in patient therapy, and follows up on problems.
- Utilizes clinical data to evaluate physician orders and ensure appropriate therapy.
- Provides and documents drug information and patient counseling.
- Documents interventions, adverse drug reactions (ADRs), and medication errors.
- Directs and utilizes personnel including technicians, aides, and clerical staff.
- Assists in monthly inspections of patient care units and the Pharmacy.
- Maintains adequate medication supply in individual work areas.
- Participates in Pharmacy staff in-services and meetings related to Systemwide and/or healthcare setting programs.
- Operates and troubleshoots automated dispensing systems as needed.
- Advises prescribers on alternatives for non-formulary drugs.
- Utilizes Outpatient and Inpatient computerized systems to enter and access patient care information.
- Coordinates and analyzes statistics and/or clinical data.
- Participates in quality management and utilization review programs.
- May coordinate the work of a small group to accomplish projects or assignments.
- May participate in the preparation and review of policies and procedures.
- May assist in the development, implementation, and execution of Pharmacy staff training curriculum.
- In the absence of the supervisor, may assist in the supervision of a Pharmacy unit or function.
- Supervises and/or coordinates the operation of a Pharmacy unit or major function.
- Prepares departmental reports, including statistical analyses.
- Develops and implements new processes to improve Pharmacy operations and quality of care.
- Orients new staff members and monitors/evaluates staff performance.
- Prepares unit work schedules and ensures compliance with regulatory agency rules.
- Manages adequate quantities of medication and supplies in the Pharmacy unit.
- Evaluates new FDA-released drugs for efficacy, adverse effects, and cost; makes formulary recommendations.
- Manages medication therapy for a group or unit of patients as part of an interdisciplinary team and participates in rounds.
- Coordinates the assessment of medication use and serves as a professional resource in specialty area(s).
- Initiates and/or participates in research studies; conducts drug administration aspects of drug research projects and prepares reports.
- Provides pharmaceutical education to professional staff at lectures, meetings, or individually.
- Coordinates activities of other supervisors in integrating functions and staff activity of multiple Pharmacy units.
Requirements
- For Level I: Possession of a valid NY State license to practice pharmacy.
- For Level II: In addition to Level I requirements, two years of post-licensure experience; or completion of a postgraduate specialty residency or fellowship program; or current Pharmacy Board Certification.
- For Level III: In addition to Level II requirements, one year of post-licensure supervisory experience or specialty clinical experience.
