Health Support Specialist

About the role

The Health Support Specialist works under the supervision of the Residential Manager and Registered Nurse, and in cooperation with a person's medical and psychiatric providers. This position assists people enrolled in The Arc of Rensselaer County programs to maintain good health and well-being. They will also support the residential team with managing health care appointments, maintaining health records, and playing a key role in the implementation of nursing plans of care.

Responsibilities

• Aid in maintaining good health and well-being of individuals enrolled in The Arc of Rensselaer County programs.
• Support the residential team with managing health care appointments.
• Maintain health records.

Requirements

• High School Diploma or GED.
• 2 years of experience working in human services.

Qualifications

Requires a HS Diploma or GED and 2 years of experience working in human services.
