Health Risk Assessor - NP/PA (Flexible Schedule)

About the role

Perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members. Visits involve patient interaction, health-information gathering, and assessment completion; no treatment or prescriptions are provided.

Responsibilities

• Conduct in-person patient interactions during home visits
• Gather comprehensive health information
• Complete accurate Health Risk Assessments
• Ensure thorough and precise documentation of all findings

Requirements

• Master’s degree in Nursing or Physician Assistant studies
• Active Nurse Practitioner or Physician Assistant license
• Relevant clinical certifications
• Ability and willingness to travel to patient homes
