About the Role
The County of Monterey Health Department is seeking a motivated individual to fill one (1) Health Program Coordinator (HPC) position within the Public Health Bureau’s Enhanced Care Management Program (ECM). This is a full-time, regular, staff-level professional position reporting to the Public Health Program Manager I.
The Enhanced Care Management Program serves clients who are experiencing homelessness, have chronic medical conditions, have avoidable emergency department and/or hospitalization usage, have severe mental health diagnosis or challenges with substance use, and pregnant and postpartum individuals who are subject to racial and ethnic disparities. The Health Program Coordinator will specialize in knowledge of 1-2 of these populations.
The HPC will hire and train new team members, assign and review the workload of the professional team, and coordinate with partners. The role includes direct supervision of professional staff such as Community Service Aides II, Behavioral Health Aide, and/or clerical staff. The HPC evaluates program effectiveness to ensure ECM’s daily operations comply with long- and short-term program goals, agency policies, and procedures.
The Eligible List established by this recruitment will be used to fill current and future vacancies, both regular and temporary, as they arise.
Responsibilities
- Organizes, supervises, and monitors specialized health-related services and activities.
- Answers questions and provides information to the public; supervises the enforcement of community health-related standards, codes, and regulations, as required.
- Selects, supervises, trains, and evaluates professional, technical, and clerical subordinates.
- Participates in the development of goals, objectives, policies, procedures, and priorities within a specialized service area.
- Prepares cost estimates and justifications for budget recommendations; assists in monitoring and controlling expenditures to ensure objectives are met within an approved budget.
- Maintains effective working relationships with public and private organizations, including other department divisions, County departments, and outside agencies.
- Provides staff assistance to higher-level management on program issues.
- Develops and maintains effective working relationships with service and healthcare providers and community groups.
- Plans, coordinates, maintains, and evaluates programs; engages stakeholders from the community and potential partnership organizations regarding prevention, detection, and response to public health issues.
- Develops program goals and objectives with stakeholder input.
- Uses principles of reflective practice and staff development to provide primary support and oversight to home visitors, building and modeling a strengths-based, client-centered culture.
Requirements
The successful candidate will demonstrate the following knowledge, skills, and abilities:
- Thorough knowledge of principles and practices of public administration and program management, including program planning, implementation, and evaluation.
- Principles and practices of community health care.
- Principles of budget preparation and monitoring.
- Principles of personnel training, supervision, and evaluation.
- Skill and ability to apply principles and techniques of community organization, including community awareness, resource coordination, and mobilization of diverse community groups.
- Organize, direct, and coordinate program services.
- Provide excellent and courteous customer service and establish and maintain effective working relationships.
- Analyze problems accurately, develop recommendations, and take appropriate action to resolve them.
- Prepare complex and detailed written reports, procedures, and contracts.
- Speak effectively to diverse audiences, including professional, clinical, and citizen groups.
- Train staff and coordinate activities of contractors, volunteer groups, and staff.
Conditions of Employment
- Successfully complete a background check, including fingerprinting.
- Possess and maintain a valid California Class C driver’s license or provide suitable transportation approved by the appointing authority.
- Be available to work a flexible schedule, including evenings, weekends, holidays, and during times of disaster and/or emergency.

Pay
$7,784 – $10,633 per month
