Health Navigator (Bilingual-Spanish)
Major Functions And Accountabilities
- Contact designated members after ER discharge and assist in following up with their Primary Care Provider (PCP) and other medical appointments.
- Aid members with health education services both within the health plan and in community health education resources.
- Aid members in accessing necessary services and educate them on the concept of a medical home.
- Refer members needing care management to the health services department.
- Maintain a tracking and evaluation system for all contacts and follow-ups.
- Increase awareness and education among members about using their PCP and when to visit the ER.
- Collaborate with hospital and clinic staff to educate them about the Health Navigator Program and its goals.
- Prepare reports and perform other duties as needed.
Experience, Training, And Qualifications
Education: Possession of a baccalaureate degree (BA) from an accredited college or university with a major in: community health promotion, health education, sociology, psychology/health psychology or closely related field.
Experience: Experience in working with low-income, diverse populations; experience in the healthcare field including hospitals.
Other: Other medical training or experience and other certifications may be evaluated on a case-by-case basis.
Knowledge, Skills, And Abilities
Knowledge of Managed Care and health care principles; knowledge of project planning and evaluation techniques; strong understanding of cultural needs of diverse ethnic minority populations and sensitivity to community and cultural groups; ability to relate to other professionals and prepare reports and correspondence.
