Health Navigator
About the role
The Health Navigator's role is to make navigating the changes to the healthcare system easier for both doctors and patients. You will develop strong relationships with patients to assist them with their care, meet with patients during the check-in process, enter data into the Main Street software platform, order and schedule various procedures, tests, and screenings, identify and track important gaps in patient care, maintain a record of patient interactions, communicate with providers using electronic health records, educate patients on their healthcare options, insurance benefits, and common medical conditions, help patients access various community resources, and coordinate follow-up care after patients have been discharged from the hospital. Demonstrating compassion, adaptability, attention to detail, patience, and an eagerness to collaborate with team members is essential.
Responsibilities
- Develop strong relationships with patients to assist them with their care
- Meet with patients during the check-in process, entering data into the Main Street software platform for reporting and tracking purposes
- Call patients to schedule an office visit or remind them to pick up their medication
- Order and schedule various procedures, tests, and screenings
- Identify and track progress on important gaps in patient care
- Maintain a record of patient interactions and communicate with providers using electronic health records
- Educate patients on their healthcare options, insurance benefits, and common medical conditions
- Help patients access various community resources
- Coordinate follow-up care after patients have been discharged from the hospital

- An active unencumbered driver's license is required
