AuthoraCare Collective is seeking a Health Information Specialist to maintain, update, monitor, and secure medical records while ensuring compliance with organizational, state, federal, and insurance regulations.

About the role

This is a full-time hybrid position with a schedule of Monday through Friday, 8:00 AM to 5:00 PM.

Responsibilities

  • Audits for compliance and accurately indexes all patient-related forms into the electronic medical record (EMR).
  • Audits patient admissions, recertifications, transitions in care, level of care changes, and deaths/discharges/revocations/transfers for compliance with organization, state, federal, and insurance regulations.
  • Processes, sends, and resends all medical orders and plans of care to the appropriate provider within expected timeframes and in accordance with regulations.
  • Audits for compliance and indexes signed medical orders and plans of care into the EMR.
  • Manages new attending provider signature logs to ensure compliance.
  • Audits external attending and internal providers’ licensure, PECOS enrollment, and other eligibility criteria within expected timeframes.
  • Utilizes external software (e.g., Vynca, Epic, NC Dave) to maintain fluid patient data exchange for continuity and quality of care.
  • Participates in medical records phone line rotation to respond to internal and external health information requests or concerns.
  • Provides direction and monitors work for office volunteers to ensure accuracy and completion of assigned duties.
  • Responds to and fulfills internal and external medical record requests in accordance with organizational policies, compliance, and timeliness expectations.
  • Prepares necessary data for hospice interdisciplinary team (IDT) meetings in accordance with organizational policies, compliance, and timeliness expectations.
  • Sends appropriate patient updates to providers in line with organizational policies, compliance, and timeliness expectations.
  • Maintains confidentiality and HIPAA compliance while working with protected health information.
  • Collaborates with other departments to complete timely health information management duties.

Requirements

  • High school diploma or equivalent.
  • Minimum of one year of experience in a position using medical secretarial skills.
  • Certification, accreditation, or associate degree in healthcare administration, medical records, or a related field preferred.
  • Medical terminology certificate preferred.
  • Valid state-issued driver’s license required.
  • Must carry automobile liability insurance at limits required by the agency.
  • Must have own transportation.
