Health/Human Service Case Manager II
About the role
The Department of Health and Social Services (DHSS) seeks a Health/Human Service Case Manager II to assist individuals and families in obtaining various health and human services benefits.
Responsibilities
- Determine initial and ongoing eligibility for all needs-based benefits including Food Supplement Benefits (SNAP), Medical Assistance, Delaware Healthy Children’s Program (CHIP), Subsidized Childcare, State Funded General Assistance (GA), and Temporary Assistance to Needy Families (TANF).
- Work closely with TANF and SNAP vendors.
- Counsel, case manage, and make referrals for benefits DSS does not provide (such as Child Support, foreclosure assistance, or food closets).
- Share information about benefits in a timely and accurate manner to fulfill the diverse needs of vulnerable and low-income residents.
Requirements
- Six months' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
- Six months' experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
- Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.
- Six months' experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.
- Six months' experience in narrative report writing.
Qualifications
- Education, training, and experience demonstrating competence in case management, recommendation-making, law interpretation, automated system usage, and narrative report writing.
Skills
- Ability to communicate and interact effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.
- Proficiency in using automated information systems and computer software to research, review, evaluate, and report on health/human service data.
- Knowledge of Delaware Code, Title 24, and related laws, rules, and regulations.

Pay
The salary range for this position is $45,000 - $55,000 annually, depending on qualifications and experience.
Schedule
This position is full-time and works standard business hours.
