HEALTH HOMES+ SUPERVISOR - Central Islip, NY

Responsibilities

• Provide care coordinators with support and guidance in managing their caseload.
• Supervise Care Coordinators as they coordinate comprehensive medical and behavioral care to clients during scheduled supervision.
• Review members' progress notes for detail and completeness.
• Review and approve members’ documentation in electronic health record.
• Coordinate/case conference with other social service networks as needed.
• Monitor program performance, productivity, quality, and compliance with Health Home Plus (HH+) requirements.
• Assume other duties as assigned by the Director for program success.

Requirements

• Valid Driver's License and insured vehicle.
• PPD test with proof.

Qualifications

• Managerial skills and knowledge.
• Computer knowledge, including but not limited to Windows and Microsoft Office.
• Ability to work as part of a team with co-workers and other colleagues.
• Cultural sensitivity and the ability to relate/work with diverse groups, community agencies, schools, and the general public.
• Excellent verbal and written communication skills, including strong interpersonal and organizational skills, focusing on detail.
• Ability to follow directions and communicate well (orally and in writing).

Education/Training/Experience

• Bachelor’s degree in an appropriate field (Social Work, Human Services, Psychology, etc.) and 3+ years’ experience providing Health Home or equivalent SMI/HARP/SED services to individuals.
• OR Master’s degree in an appropriate field (Social Work, Human Services, Psychology, etc.) and minimum 2+ years’ experience providing Health Home or equivalent SMI/HARP/SED services to individuals.
