Job Summary
Penquis, a social services agency serving Penobscot, Piscataquis, and Knox counties for over 50 years, is seeking a full-time (40hrs) Health Home Coordinator. This position is remote in terms of office/administrative work but requires travel to meet clients in Penobscot and Piscataquis counties.Responsibilities
- Work with individuals to address behavioral health needs, as well as integrating physical health and medical needs.
- Participate in a team-based approach to care by working in collaboration with your supervisor, Peer Support Specialist, Clinician, Nurse Care Manager, Psychiatric and Medical Consultants.
- Provide accurate clinical assessments and annual assessments.
- Independently track all Mainecare review dates, assessment reviews, and treatment plan due dates for a year at a time.
- Participate in ensuring the delivery of crisis intervention and resolution services, providing follow-up services to ensure that a crisis is resolved and assisting in the development and implementation of crisis plans.
Requirements
- Requires a Bachelor’s Degree in a Social Work, Child Development, Human Services, or related field.
- 2-3 years’ experience working with children, adolescents or adults with disabilities.
- Must have a valid State of Maine driver’s license in good standing and reliable insured transportation and available to work flexible hours to meet the needs of the client and families.
- Upon hire and periodically thereafter, must comply with Penquis background check requirements with satisfactory results as outlined in current contract provisions.
