Health Home Care Coordinator

About the role

Ohel is seeking a Care Coordinator to manage the care of adults enrolled in Ohel's Health Home program.

Responsibilities

• Assess the adult's physical, mental health and social services needs
• Develop an integrated plan of care, working collaboratively with medical, behavioral, educational and social service providers
• Provide care coordination and health promotion, transitional care and follow up, individual and family support, referrals to community and social support services
• Use health information technology to link services

Qualifications

• Bachelor's degree required, preferably in the Human Services field
• Experience working with individuals who have behavioral health needs such as a serious emotional disturbance, mental health challenge, intellectual disabilities, or substance use disorder is preferred

Schedule & Location

This full time position is based in Brooklyn with home and hospital visits as needed.

Pay

Bachelors Level $50,000 / Masters Level $55,000
