Health Care Surveyor 3 (Nursing) (NY HELPS) - 82742

Team Leader responsible for conducting onsite investigations and post-survey revisits to verify facility compliance with State and Federal regulations.

Responsibilities

• Prepare reports and write statements of deficiency based on investigative findings.
• Consult with interdisciplinary teams to evaluate resident care according to CMS regulations.
• Provide clinical consultation during onsite investigations and surveillance activities.
• Review investigative reports, statements of deficiency, facility information, and survey history prior to surveys.
• Participate in pre-survey team meetings and utilize gathered information to plan and implement onsite surveillance strategies.
• Engage in interdisciplinary survey discussions, arriving at decisions through team consensus.
• Develop written documents according to federal principles of documentation.
• Conduct investigations of resident and facility complaints as directed by the team leader and program managers.
• Provide direction in the appropriate interpretation and application of federal and state regulations.
• Participate in other surveillance activities, including federal and state training programs or emergency response.

Requirements

• Possession of a license and current registration as a registered professional nurse in New York State.
• Three years of post-licensure, specialized professional administrative or professional clinical experience.
• Substitution: A bachelor’s degree in nursing can substitute for one year of post-licensure clinical nursing experience; a master’s degree in nursing can substitute for two years of post-licensure clinical nursing experience.
• For Permanent Transfer Candidates: Current NYS Department of Health employee with one year of permanent or contingent permanent, competitive service as a Health Care Surveyor 3 (Nursing).
• Federal Surveyor Minimum Qualifications: Testing certification required; training certification required within one year of employment.

• Strong interview, observational, and writing skills, with the ability to translate collected information into legally defensible reports.
• Demonstrated experience in oversight of quality assurance and making determinations relative to facility compliance.
• Ability to develop effective and efficient processes and supervise staff.
• Flexibility and dedication to improving outcomes for aged and aging New Yorkers.
• Experience with the aged and aging population, particularly in direct patient care as a human services or clinical professional.
• Understanding of how the aging process affects outcomes.

Additional Information

• Position requires travel to areas often underserved by public transit, including overnights throughout New York State.
• Flexible availability to work varied hours, which may include weekends and holidays, to respond to emergencies or off-hours investigations.
• Work hours will be determined at the time of hire.

Pay

Posted salary range includes the statutory hiring rate at Step 4 and job rate for PS&T Grade 22 ($93,178 - $104,230) plus an additional Geographic Pay Differential of $35,000 and downstate adjustment of $4,000.
